The Polk County Health Department is located on the west side of the Courthouse Square in Osceola, Nebraska.  Office hours are 8:00 a.m. to 4:30 p.m. Monday through Friday.

General Information about Polk County Health Department:

The Polk County Health Department has been in existence since 1979.  The services offered are available to all residents of Polk County, regardless of income status.  Funding comes from a number of sources including:  private contracts, private payment based on a fee for services, donations, grants and tax support from the Polk County Government.
